To illustrate the critical radiographic characteristics of a successful iliac crest free flap, we present a review of five patients who underwent palatomaxillary reconstruction using this type of flap. Radiographic characteristics presented can function to improve imaging interpretation and prevent false identification of tumor recurrence.
