Head and neck bony reconstruction often utilizes the fibular free flap (FFF). Although uncommon, ossification (abnormal bone formation) of the vascular pedicle is a reported complication. This paper presents a patient whose PET/CT imaging showed FDG avid free flap ossification, a finding not present in the current literature.
